Construct a WCS 2.0.1 elevation coverage (cache-agnostic).
WCS 2.0.1 needs the per-coverage DescribeCoverage XML to determine bounding box / axis labels / level count before the coverage object can exist. serviceMetadata (caller-supplied XML) is used verbatim when non-null; otherwise the factory issues a DescribeCoverage request.
